Neighborhood PACE enables adults 55+ to stay healthy, active, and independent in the place they call home. Our participants often tell us they have never been as active or happy!

Neighborhood PACE serves adults living in East Boston, Chelsea, Winthrop, Revere, Everett, Malden, Melrose, Medford, Stoneham, and Boston’s North End. As part of the national Program of All-Inclusive Care for the Elderly (PACE) network, we provide the comprehensive care and support that you need as an individual with specific needs and preferences.

Our program is the ideal solution for those who want an alternative to nursing home care for the right combination of medical, social, recreation, rehabilitation, home care, and other services. We even provide medications without co-pay and transportation to each PACE Center and medical appointments.

Neighborhood PACE is supported through a PACE program agreement with the Centers for Medicare & Medicaid Services, MassHealth, and the East Boston Neighborhood Health Center. Neighborhood PACE is comprehensive. Neighborhood PACE participants must receive all necessary health care, including primary care and specialist physician services (other than emergency services), from Neighborhood PACE or from an entity authorized by Neighborhood PACE. Participants may be fully and personally liable for the costs of unauthorized or out-of-program services. You may request disenrollment from Neighborhood PACE at any time. Your effective date of disenrollment will be the first day of the month following receipt of your request. Please be aware that you cannot unenroll from Neighborhood PACE at a Social Security office.

Each PACE Center provides integrated medical care, recreation, socialization, rehabilitation, and therapeutic recreation all in one community-centered location. Participants can spend the day in a warm and caring atmosphere filled with friends and engaging activities.

The PACE model of care evolved from the belief and experience that older adults with chronic care needs, and their families and caregivers, are better served by remaining in their homes and community whenever possible.

PACE began as a demonstration project in the early 1970s in San Francisco. On Lok (Cantonese for “place of peace and happiness”) Senior Health Service coordinated many community-based services to help families in the Italian-Chinese North Beach neighborhoods care for aging family members — and avoid a nursing home placement. Ultimately this model of care for older adults was funded by the Centers for Medicare and Medicaid Services and adopted in many other states.
